How to Replace your Social Security Card or Medicare Card
To replace a Social Security card: http://www.socialsecurity.gov/ssnumber/ Original documents (State ID, Passport, etc.) can be mailed to the local Social Security office if an actual visit is inconvenient or impossible. Otherwise, the application for a replacement card must be done in person. To replace a Medicare card online: https://secure.ssa.gov/apps6z/IMRC/main.html Both of these requests can be completed during a visit to the Social Security office. The Medicare card request can also be completed via phone at (800) 772-1213. |
SHINGLES Vaccine Information
Medicine is stored in a frozen state, therefore your desired pharmacy will need to have the capability to deliver the drug to your doctor's office where you will need to set up an appointment to have the vaccine administered. A script is required. For most plans the vaccine (Zostavax) is either a tier 2 or tier 3. Check with your individual plan for exact cost....or call your trusted agent!!! Shingrix is a newer vaccine for shingles. It is, usually, a tier 3 on your Medicare Prescription drug plan. You will need a series of 2 shots to complete this vaccine.
